Troy, HA usually happens when these possible conditions are not met: 1) adequate skimming (need to check if your skimmer size comparing with your tank water volume, gallons). 2) overfeeding. 3) not using ro/di water 4) not enough water flow, need to increase water circulation inside the tank and through the skimmer. underskimming, no water change for 3 months and put in a few aptasia to start growing. How did I find that out? Been busy with new business for 3 months and just kill all of them the last two days with shooting kalkwasser. Good thing this aptasia only happens in my frag/hospital tank. Not sure how to get to all of them if it's in the larger tank.

It is a myth on overskimming. No such thing. Buy the most efficient and best skimmer that your wallet can afford. Don't skim on skimmer.
