I looked. Locating a file from 12 years ago seemed to be an impossible task. However, I was able to log into the OP's account (he's a former WAMAS membership director) and found the attachment. Here's the page. It's pretty much just an old-style log book page. I'd suggest that you look at other options more in-line with current technologies - such as a reefkeeping log book app or cloud app that can do the job. I'm sure that something better is out there.
