I think you've just answered your own question. You have a large tang in a 34g tank. He's probably stressed because of the lack of space to swim. I don't know if that is the cause of the dorsal fin issue, but it's not a good thing. I would sell him to someone with a big enough tank for him.

Or, you could always upgrade to a bigger tank!!

Wow... it amazes me the size tanks that tangs get put into. I feel bad for my sailfin tang that is in my 220 gallon because she is TOO BIG for it. A yellow tang in less than a 50 gallon, I imagine, is enough to kill it or at least make it sick. Tangs swim alot and need room.

Sell the fish or get it a bigger tank! Also make sure you are feeding it algae sheets/nori frequently.

When I had my Purple Tang in my 10G QT, his fins were always extended. Then he'd twitch them all the time too like he was antsy. I thought it was ammonia, lack of O2, etc, but as soon as he got into the 125, he was all normal again.

Yup. Tangs need space. I had a nice Yellow-Belly Hippo Tang but had to give him up since he was getting too big for my 100 gal tank. His tank mates do NOT miss him! Now, the yellow tang (and sometimes the flame angel) get to play the role of the Alpha fish.
