Hammer coral issue

What is going on with my hammer coral I have had it for 4 years now and it looks like it is going to die

Parameters 

Calcium 550

Alkalinity 6

Nitrate 0

Phosphate .50

Look up KFC dip. If it's not too late you maybe able to save it. By the way, your Alk seems a bit on the low side. They also like nitrate, looks like your nitrate is bottomed.
