IME bleaching is usually caused by too much light.  Especially in tanks lit by LEDs, because they put out a lot of light but don't appear that bright.  A spike in water temps can also cause bleaching.

Move your coral into a partially shady spot, and consider turning down your light intensity.

WHAT CHANGED BEFORE BLEACHING?

When I see corals in trouble and assuming there were no obvious changes, I just go ahead and do a 40-45% water change? Half the time this solves problem.

What happens the other 55 or 60% of the time?  Do the corals turn into wood?
